A specialized opportunity is available for a Sterile Compounding Pharmacist with expertise in home infusion pharmacy practices. This contract role spans 13 weeks and is based in San Antonio, TX, with a consistent 12 PM to 8 PM schedule. The position focuses on evaluating patients for home infusion therapy, developing personalized medication plans, and ensuring the safe preparation and dispensing of sterile medications.

Key Responsibilities:

  • Assess patients to determine eligibility for home-based infusion therapy.
  • Develop and adjust individualized medication therapy plans using clinical assessments and pharmacokinetics.
  • Prepare, verify, and dispense sterile medications in strict adherence to USP standards and regulatory requirements.
  • Educate patients, caregivers, and healthcare providers on medication use and infusion administration.
  • Collaborate with healthcare teams to ensure continuity of care and monitor patient outcomes.
  • Manage infusion pumps and medication administration equipment while upholding quality assurance and safety protocols.
  • Provide drug information, maintain accurate documentation, and support medication safety initiatives.
  • Qualifications and Desired Experience:

  • Doctor of Pharmacy (PharmD) degree or graduation from an accredited School of Pharmacy.
  • Active, unrestricted Texas Pharmacist license.
  • One or more of the following:
  • Minimum one year of sterile IV compounding experience.
  • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
  • Completion of an ASHP-accredited Pharmacy Residency.
  • Completion of an Advanced Pharmacy Practice Experience (APPE).
  • Equivalent pharmacy experience.
  • Strong clinical knowledge of home infusion pharmacy practices.
  • Proficiency in interpreting laboratory values and applying pharmacokinetic principles.
  • Excellent communication, patient counseling, and customer service skills.
