Position Overview
Seeking a Clinical Pharmacist to support medication management, clinical consultations, and patient care initiatives. This role involves medication therapy reviews, formulary management, disease state management, and collaboration with healthcare providers.
Responsibilities
• Conduct medication therapy reviews and drug utilization analysis.
• Support formulary management and disease state management.
• Provide clinical consultation and medication education to patients and providers.
• Lead and participate in Pharmacy & Therapeutics (P&T) meetings.
• Support regulatory compliance and medication safety initiatives.
• Collaborate with healthcare providers and clients.
• Attend administrative meetings and occasionally travel to client sites.
Requirements
• Pharm.D. degree.
• Active pharmacist license (Maryland preferred).
• Must have completed a PGY1 residency or hold a board certification.
• BCPS or BCACP certification preferred.
• HIV therapy management experience preferred.
• CPR certification preferred.
