Staff Pharmacist | Mesa, Arizona

A home infusion clinical pharmacist position is available for an immediate start on a contract basis lasting 8–12 weeks. This role is based in Mesa, AZ, with a Monday to Friday schedule from 8:30 a.m. to 5:00 p.m., and may include participation in an on-call rotation.

Responsibilities include preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a sterile environment, following all applicable state and federal regulations. The pharmacist serves as a key member of a multidisciplinary team, providing clinical monitoring by assessing lab values, calculating pharmacokinetic dosing, and adjusting Total Parenteral Nutrition (TPN) treatments. Additionally, patient and caregiver education on infusion pump use, sterile techniques, and catheter maintenance is a vital component of this role.

Collaboration with prescribing physicians, home health nurses, and various pharmacy operation teams ensures seamless therapy management and helps prevent hospital readmissions. Compliance with regulatory standards such as HIPAA, DEA, Medicare, ACHC, URAC, OSHA, and State Board of Pharmacy regulations is required. The role also involves managing pharmacy inventory, including refrigerated products, and coordinating the packing and shipping of medications and supplies.
