Job Title: Inpatient Hospital Staff Pharmacist
Location: Chicago, Illinois
Shift: Rotating first, second shift: 7am – 3:30pm; 3pm – 11:30pm, rotating weekends

Job Description:
RPh on the Go is seeking a highly skilled and dedicated Inpatient Hospital Staff Pharmacist to join our clinical team in Chicago, IL. This position involves providing comprehensive pharmaceutical care to hospitalized patients, ensuring safe and effective medication therapy. The role includes rotating 1st and 2nd shifts, as well as rotating weekend coverage. As a key member of the healthcare team, the Inpatient Staff Pharmacist will work closely with physicians, nurses, and other healthcare professionals to optimize patient outcomes.

Key Responsibilities:
•    Accurately review, interpret, and dispense medications for hospital inpatients in accordance with physician orders.
•    Provide clinical pharmacy services, including medication therapy management, drug utilization reviews, and therapeutic drug monitoring.
•    Collaborate with healthcare providers to ensure safe and effective medication regimens, making therapeutic recommendations when appropriate.
•    Monitor patient responses to medications and adjust therapy as needed based on clinical guidelines and patient conditions.
•    Educate patients, caregivers, and hospital staff on medication usage, potential side effects, and drug interactions.
•    Ensure compliance with all hospital policies, procedures, and regulatory requirements, including Joint Commission standards.
•    Oversee and support pharmacy technicians in the preparation and dispensing of medications.
•    Participate in hospital committees, clinical rounds, and multidisciplinary care teams as needed.
•    Maintain accurate patient medication profiles and documentation in the hospital’s electronic health record (EHR) system.
•    Support hospital operations through inventory management, including ordering and restocking medications as needed.
•    Assist in maintaining the sterile compounding area and ensure compliance with USP <797> and <800> standards.

Qualifications:
•    Doctor of Pharmacy (Pharm.D.) from an accredited institution.
•    Active and unrestricted Illinois Pharmacist License or eligibility to obtain licensure before the start date.
•    2+ years of experience in a hospital or clinical pharmacy setting preferred.
•    Strong knowledge of pharmacokinetics, pharmacotherapy, and medication safety standards.
•    Proficiency in using electronic health record (EHR) systems and pharmacy management software.
•    Ability to work effectively in a fast-paced, multidisciplinary hospital environment.
•    Excellent communication and interpersonal skills, with a focus on patient care.
•    Strong organizational and time management skills.
