RPh on the go is searching for Pharmacist. The ideal candidate will have a year or more LTC /retail experience.

Shift: M-F, Mid Shift (11am - 7:30pm). 32 hours/week

Key Responsibilities:

  1. Medication Management:

  2. Review and analyze patient medication profiles to ensure appropriate, safe, and effective medication use.
  3. Monitor and evaluate medication regimens for therapeutic efficacy, potential side effects, and drug interactions.
  4. Recommend adjustments to medication therapy as needed, based on patient response and clinical guidelines.
  5. Patient Care:

  6. Conduct medication therapy management (MTM) and medication reconciliation for new admissions and periodic reviews.
  7. Provide drug information and education to patients, families, and caregivers on proper medication use and potential side effects.
  8. Collaborate with healthcare teams, including physicians, nurses, and other staff, to optimize patient care plans.
  9. Compliance and Documentation:

  10. Ensure compliance with federal, state, and local regulations regarding medication management and documentation.
  11. Maintain accurate and up-to-date records of medication orders, changes, and patient responses.
  12. Assist with audits and inspections to ensure adherence to regulatory standards.
  13. Quality Assurance:

  14. Implement and monitor quality improvement initiatives related to medication use and patient safety.
  15. Develop and review policies and procedures related to medication management and pharmacy services.
  16. Education and Training:

  17. Provide training and support to facility staff on medication-related topics, including new drug therapies and best practices.
  18. Stay current with advancements in pharmacotherapy and long-term care practices through continuous education and professional development.
  19. Pharmacy Operations:

  20. Oversee the procurement, storage, and dispensing of medications within the facility.
  21. Ensure proper handling and disposal of medications, including controlled substances.
  22. Coordinate with pharmacy vendors and suppliers to ensure timely and accurate medication

Required

  • Pharmacy experience. 
  • LTC experience a plus.
