We are currently seeking a dedicated Patient Care Representative (PCR) to join our Pharmacy Operations team in Nashville, TN. Job Summary:
Under the supervision of a Pharmacist or Pharmacy Manager, the Patient Care Representative is responsible for communicating with patients, ensuring accurate medication profiles, coordinating therapy follow-ups, and managing medication refills. This role will also involve assisting pharmacists with obtaining laboratory values and maintaining inventory for patients.
Roles and Responsibilities:

  • Patient Communication: Follow up with patients to review medication inventory and ensure timely refills.
  • Documentation: Maintain accurate and timely documentation in patient progress notes and EMR systems.
  • Collaboration: Communicate with patients, caregivers, and clinicians to discuss therapy progress, address concerns, and relay clinical questions to the pharmacist.
  • Inventory Management: Anticipate and address patient supply needs to maintain adequate stock of necessary medications and equipment.
  • Physician Follow-Up: Follow up with physician offices regarding changes in patient status, prescriptions, and lab results.
  • Compliance: Ensure compliance with HIPAA and confidentiality regulations regarding patient information.

    Qualifications and Experience:
    Required:

  • Active Tennessee Pharmacy Tech license
  • Exceptional communication skills (both verbal and written).
  • Strong organizational skills and ability to manage multiple tasks simultaneously.
  • Ability to work under pressure, meet deadlines, and handle a fast-paced environment.
  • Proven critical thinking and problem-solving abilities.
  • Strong customer service skills with a track record of customer satisfaction.

    Preferred:

  • Minimum of 6 months pharmacy technician or related experience.
  • Working knowledge of pharmacy procedures, including Home Infusion, Specialty Infusion, and Durable Medical Equipment (DME).
  • 2+ years of computer data entry or related experience.
