This position is for an advanced practice pharmacist specializing in oncology at a high-performing oncology pharmacotherapy clinic. You will work directly with patients and healthcare teams, improving patient outcomes by managing medication therapies for complex diseases such as cancer. You will be a key player in optimizing care plans, simplifying drug regimens, and educating patients and caregivers to ensure adherence and maximize clinical outcomes.

Key Responsibilities:

  • Collaborative Practice: Initiate, modify, and discontinue medications under a Collaborative Practice Agreement with physicians.
  • Complex Case Management: Focus on managing complex oncology disease states, tailoring individualized therapeutic plans to help patients achieve their health goals.
  • Patient Education: Provide in-depth patient education to improve medication adherence, understanding, and long-term outcomes.
  • Medication Optimization: Streamline medication regimens to minimize drug interactions and improve safety.
  • Clinical Care & Assessments: Perform patient assessments, order and interpret labs, and modify therapy as needed based on clinical outcomes.
  • Continuous Process Improvement: Lead in the development of clinic policies and continuous process improvements.
  • Leadership & Development: Serve as a mentor and preceptor for Pharmacy APPE students and residents, participate in staff education, and support medical staff development.
  • Documentation: Ensure accurate and timely electronic health record documentation, and communicate with other healthcare providers to ensure continuity of care.

    Ideal Candidate Qualifications:

  • PharmD with PGY1 + PGY2 specialization in oncology or at least 4+ years of current oncology-focused ambulatory care experience.
  • BCOP (Board Certified Oncology Pharmacist) certification is preferred.
  • Strong experience in managing oncology patients in a clinical setting, ensuring they reach optimal therapeutic outcomes.
  • Ability to develop and implement medication management protocols in a high-stakes, dynamic oncology environment.
  • Must be able to obtain a California Pharmacy License.
