Position Title: Hospital Pharmacist
Location: Coquille, Oregon
Employment Type: 1st shift, 8:00am – 4:00pm

Position Overview:
RPh on the Go is seeking a dedicated and knowledgeable Hospital Pharmacist to join our pharmacy team. The ideal candidate will be responsible for the accurate preparation, dispensing, and monitoring of medications for hospitalized patients, ensuring compliance with established policies and best practices. This role is essential in promoting the safe and effective use of medications, working closely with healthcare providers, nurses, and other interdisciplinary teams.

Key Responsibilities:
•    Medication Management: Accurately prepare, compound, and dispense medications, including sterile products and IV admixtures, in accordance with physician orders.
•    Clinical Consultation: Provide expert consultation to healthcare professionals regarding drug therapy, including pharmacokinetics, dosing, interactions, and side effects.
•    Patient Safety: Ensure proper medication storage, handling, and disposal, while adhering to regulatory standards (e.g., Joint Commission, State of Oregon, and federal guidelines).
•    Medication Reconciliation: Participate in the medication reconciliation process, ensuring accurate and complete medication histories for admitted patients.
•    Pharmacy Operations: Collaborate with pharmacy technicians and other staff to ensure efficient operation of the pharmacy department, including inventory management and restocking of medications.
•    Continuing Education: Stay updated with advancements in pharmaceutical practices, participate in hospital in-service programs, and maintain Oregon state licensure.
•    Documentation: Accurately document medication orders, adjustments, and patient consultations in the hospital's electronic medical record (EMR) system.

Qualifications:
•    Education: Doctor of Pharmacy (Pharm.D.) from an accredited institution.
•    Licensure: Current, unrestricted Oregon Pharmacist license.
•    Experience: Minimum 1-2 years of experience in a hospital or clinical setting preferred.
•    Certification: Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification (preferred).
•    Skills: Strong knowledge of pharmacology, clinical judgment, excellent communication, and teamwork skills. Familiarity with EPIC or other EMR systems is an advantage.
