Soliant Health is seeking a clinically driven and motivated pharmacist to join a dynamic healthcare team in the Greater State College, Pennsylvania area. As a Staff Pharmacist, you’ll play a vital role in patient care through direct collaboration with healthcare professionals and active participation in medication therapy management. The position provides the opportunity to rotate between the central pharmacy and ambulatory care settings, offering a well rounded experience in both acute and outpatient care environments.
Key Responsibilities:
  • Accurately review and verify medication orders, ensuring appropriate therapy aligned with patient-specific needs.
  • Dispense medications safely, efficiently, and in compliance with established protocols.
  • Serve as a trusted resource for drug information, offering timely and accurate guidance to clinicians and staff.
  • Monitor and evaluate drug therapy effectiveness using the pharmacy computer system and clinical surveillance tools.
  • Oversee and participate in sterile compounding operations, ensuring proper aseptic techniques for high-risk medications, narcotic infusions, and cytotoxic chemotherapy.
  • Engage in pharmacist-managed consults and drug therapy monitoring protocols.
  • In the ambulatory care setting, contribute to patient care through collaborative practice agreements, supporting chronic disease management and therapy optimization.
  • Maintain a high standard of clinical documentation and communication across the care continuum. Qualifications:
  • Active and clear Pennsylvania pharmacist license required.
  • Minimum of 1 year of hospital or health system pharmacy experience required.
  • Strong clinical acumen and comfort with both inpatient and ambulatory care responsibilities.
  • Demonstrated experience with sterile compounding, including chemotherapy and high-risk preparations, required.
  • Prior experience or training with Meditech is strongly preferred.
  • Excellent communication, time management, and collaborative skills are essential.
