Soliant Health is seeking a dedicated and experienced Clinical Pharmacist to join a dynamic hospital team in the State College, PA area. This is an outstanding permanent opportunity for pharmacists who thrive in high-acuity environments and are looking to broaden their clinical scope.


In this role, the Clinical Pharmacist will work directly in both the Emergency Department (ED) and Intensive Care Unit (ICU), offering a unique chance to practice across multiple critical care areas. Unlike larger institutions where pharmacists may be restricted to a single unit, this position allows for true clinical growth by rotating between key departments and participating in critical patient interventions.
 

Key Responsibilities:

  • Provide direct clinical support in both ED and ICU settings
  • Enter and verify medication orders, conduct thorough medication profile reviews, and ensure accuracy and appropriateness of therapy
  • Deliver evidence-based clinical services tailored to the critical care and emergency patient populations
  • Actively participate in multidisciplinary rounds and collaborate with physicians, nurses, and other healthcare professionals
  • Respond to acute patient situations including cardiac arrest, stroke, trauma, and sepsis
  • Participate in and support the hospital’s antibiotic stewardship program
  • Perform drug therapy monitoring and manage clinical consults per protocol

    Schedule: Variable days and afternoon shifts (8-hour shifts, primarily during the day)
    Technology: Meditech
     

    Requirements:

  • BS in Pharmacy or PharmD
  • PGY1 residency or minimum of 3 years of current hospital clinical experience in acute care/critical care settings
  • Active Pennsylvania pharmacist license or eligibility to obtain prior to start
  • Strong clinical decision-making, communication, and collaboration skills
