Soliant Health is seeking an experienced and dynamic Pharmacist to take on a critical interim opportunity in a fast-paced and rewarding Home Infusion setting. We’re looking for a licensed professional ready to step into a 13 to 16-week maternity leave coverage role in Brentwood, TN. Position Overview:

As a Home Infusion Staff Pharmacist, you will play a vital role in the clinical and operational functions of the pharmacy. You’ll collaborate closely with other healthcare professionals to ensure the accurate dispensing of prescriptions, maintain regulatory compliance, and provide expert drug-related consultation. This is a full-time position with shifts running from 8:30 AM to 5:00 PM, with occasional rotation to a 9:30 AM to 6:00 PM shift when on-call coverage is needed.

Key Responsibilities:

  • Review and approve prescriptions prepared by pharmacy technicians in accordance with State Board of Pharmacy regulations.

  • Conduct medication assessments and review patient medical histories to ensure proper therapy.

  • Perform final verification of all prescriptions to guarantee accuracy and timely delivery.

  • Collaborate with the Director of Pharmacy to maintain compliance with state and federal laws, as well as company protocols—particularly around controlled substances and legend drugs.

  • Implement quality assurance and cost-saving measures for dispensing and procurement activities.

  • Serve as a resource to both internal staff (technicians, nurses) and external providers by responding to clinical and pharmaceutical inquiries.

  • Interpret and assess laboratory data, ensuring therapeutic drug levels are appropriate.

  • Step into a leadership role as needed, especially in the absence of the Pharmacist in Charge.

  • Participate in an on-call rotation to ensure continuous patient care support.

  • Maintain full compliance with HIPAA regulations and uphold confidentiality standards with patient and company data.

  • Complete required competency verifications upon hire and annually thereafter to demonstrate adherence to pharmacy best practices and regulatory standards.

    Qualifications:

  • Graduate of an accredited school of pharmacy

  • Active and unrestricted pharmacist license in Tennessee (or ability to obtain prior to start)

  • Prior experience in a home infusion or clinical setting is required

  • Strong understanding of sterile compounding and infusion protocols

  • Solid clinical acumen with the ability to assess drug interactions, therapeutic drug ranges, and patient-specific laboratory data

  • Excellent communication skills and a team-focused mindset

  • Proficiency with pharmacy software and electronic health record systems

  • Ability to work efficiently in a high-pressure, multitasking environment
