Clinical Staff Pharmacist- East Palo Alto, CA Bilingual

RPh on the go, has an immediate opening for a Clinical Pharmacist that is available to work 32 hours a week to cover a maternity leave for us at one of our clients near Menlo Park, CA.   Pharmacist must be able to speak Spanish and English, pharmacist must have knowledge of diabetes medication. 

Requirements
CA Pharmacist license
1-2 years of diabetes medication experience
Bilingual/ Spanish. English speaking 

Duties: Assess the drug therapy needs of patients & monitor progress of therapy through consultations, comprehensive medication reviews, patient education and clinical laboratory monitoring.
Assess patients for medication adherence barriers and develop a plan to maximize patient’s medication success through improving or maintaining adherence to medications.

Industry: Healthcare; Community Hospital 
Hours: 32 Monday to Thursday day shift 7am-7pm (8hour shifts)
