Position Overview:
Soliant Health is actively seeking a Clinical Pharmacist to join the inpatient team at a well-established hospital in Cleveland, OH. This role involves working closely with healthcare providers to deliver patient care services, monitor pharmacokinetic dosing, and supervise pharmacy technicians. The ideal candidate should be flexible, able to work both early and late shifts, and comfortable rotating weekends.
Duration: 13 weeks (with potential for extension)
Shift Options:

  • 4x10-hour shifts or 5x8-hour shifts
  • Shift timings: 6:00 AM - 4:30 PM or 12:00 PM - 10:30 PM
  • Every 3rd weekend rotation required

    Key Responsibilities:

  • Inpatient Staffing: Verify medication orders, oversee dispensing, and ensure appropriate medication use.
  • Clinical Dosing: Provide pharmacokinetic services, including dosing for antibiotics, anticoagulants, and other complex therapies.
  • Collaboration with Medical Team: Work closely with physicians, nurses, and other healthcare professionals to optimize patient care and drug therapy.
  • Supervise Pharmacy Technicians: Oversee the work of pharmacy technicians, ensuring accuracy and compliance with hospital protocols.
  • Regulatory Compliance: Ensure all pharmacy activities comply with hospital policies and state/federal regulations.
  • Patient Monitoring: Assist with monitoring patient therapies, adjusting treatment plans as necessary for optimal outcomes.
  • Medication Reconciliation: Provide support with medication histories and ensure accuracy during patient admissions and discharges.

    Ideal Candidate:

  • Qualifications:
  • Active OH Pharmacist license (in good standing)
  • Minimum 2 years of inpatient clinical experience
  • EPIC experience is preferred, as this is the primary system used for order entry and patient management
  • Strong knowledge of pharmacokinetics and inpatient protocols
  • Skills Required:
  • High attention to detail, especially in medication verification
  • Ability to work under pressure in a fast-paced environment
  • Excellent communication skills for both team collaboration and patient interaction
  • Leadership capabilities, especially in supervising technicians and managing medication workflow
  • Preferred Experience:
  • Experience with complex dosing protocols, particularly for critically ill patients
  • Previous experience in a teaching hospital or trauma center is a plus
