Ambulatory Care Pharmacist Job at Pennsylvania hospital.
This is a hybrid clinical/staff role-- 60% of the time will be spent doing clinical work--40% will be spent doing staffing work.
The clinical piece of the role will focus on Population Health/Ambulatory care. In this role, the pharmacist will focus on working with patients at the health system to manage medication regimens (esp on target disease states--- ie.. Diabetes).
This role will have the potential to expand into being 100% clinical as the program expands.
Director of Pharmacy is committed to expanding clinical pharmacy services, and this could be a great opportunity for an eager clinical pharmacist with a passion for patient care willing to help grow a clinical program.
