Soliant Health is seeking a highly motivated Advanced Practice Pharmacist to join a leading ambulatory oncology clinic in Sacramento, California. This is a clinic-based role focused on oral chemotherapy management across a wide spectrum of cancer diagnoses, including hematologic malignancies. You’ll play a pivotal role in a brand-new Advanced Practice program that empowers pharmacists to practice at the top of their license in close collaboration with oncologists.

Position Highlights:

Location: Sacramento, CA (Relocation Package Available)

Setting: Ambulatory Oncology Clinic

Schedule: Full-time, weekdays only

Model: 100% patient-facing, clinic-based role under CPA

Licensure: California license not required to apply

Advanced Practice License: Can be obtained post-hire

Compensation & Benefits:

Annual Salary: $210,000–$225,000

Sign-On Bonus: $10,000

Relocation Assistance: Provided

Key Responsibilities:

Co-manage patients undergoing oral chemotherapy under a collaborative practice agreement (CPA)

Order lab tests, initiate/refill prescriptions, and monitor treatment progress

Provide ongoing toxicity assessments, lab monitoring, and patient follow-up

Perform medication reconciliations and counseling at key therapy touchpoints

Work directly with oncology physicians to develop and implement a new AP pharmacist program

Serve as a provider in a direct, one-on-one patient care model

Qualifications (Need 2 of 3 Below):

PGY2 in Oncology or PGY1 + BCOP
Board Certification: BCOP, BCACP, or BCPS
1,500 hours of clinical experience under a Collaborative Practice Agreement
